Findings
The Alleged Victim (AV) relies on the facility for care and services. AV has suffered 10 falls from September to December of 2025. The facility did not implement changes to the service plan after each fall and the current service plan does not include interventions for care staff to help with fall prevention. The facility failed to update the service plan for AV, which is a violation of resident rights and is neglect of care which constitutes abuse.
