- The Alleged Victim (AV) has a history of rashes and yeast infections. AV's care plan included skin monitoring during showers to check for rashes. AV was care planned to have as needed topical cream applied two times per day for rashes. On or about December 28, 2025, staff reported AV had a rash which appeared to have evolved into a yeast infection. AV's chest was very sore, causing unreasonable discomfort. Respondent failed to ensure staff followed the care plan and ensure that staff were applying physician ordered skin creams to ensure AV's skin integrity. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
