Findings
The Alleged Victim (AV) has a history dehydration. AV's care plan did not include pushing fluids until December 28, 2025, when an ISP was put in place. On or about December 29, 2025, AV was diagnosed with a urinary tract infection, causing unreasonable discomfort. Respondent failed to properly care plan to ensure AV stayed hydrated to avoid urinary tract infections. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
