Findings
The Alleged Victim (AV) relies on the facility for care and assistance. On or about January 16, 2026, AV was scheduled to go out of the facility for a medical appointment. AV is care planned to have a staff member from the facility accompany AV to their appointments due to AV's care needs. The facility did not have a staff member accompany AV to their appointment on January 16, 2026, and this resulted in AV experiencing pain and significant discomfort due to their care needs not being met. The facility failed to follow the care plan, which is a violation of resident rights and is neglect which constitutes abuse.
