- Alleged Victim (AV) relies on the facility for his/her care needs. On or about February 6, 2026, it was reported that the night before while AV was refusing to get up from a chair in the dining room, Alleged Perpetrator 2 (AP2) came up to AV and grabbed AV by the wrist and pulled AV out of the chair. When AP2 grabbed AV’s wrist and pulled AV out of the chair AV told AP2 to “let go.” As a result, AV reported that his/her wrist was sore on the morning of February 6, 2026. AP2 had completed the caregiver and me tech training. AP2 stated that AP2 knows not to grab and pull a resident out of a chair. AP2 failed to protect AV from physical abuse, which is neglect of care and constitutes abuse. The facility failed to protect AV from physical abuse, which is a violation of Oregon Administrative Rules.
