Findings
Alleged Victim (AV) relies on the facility for his/her care needs. Witness (W1) stated that staff had found Alleged Perpetrator 2 (AP2) sleeping inside of the facility breakroom at the time the AV was found on the floor of (AV) bedroom. AP2 stated that (AP2) set a timer at the time that (AP2) was falling asleep although the alarm did not go off and (AP2) did not know how long (AP2) had slept. AP2 failed to provide service to AV by falling asleep, which is neglect of care and constitutes abuse. The facility failed to ensure AV received service, which is a violation of Oregon Administrative Rules.
