Findings
The Alleged Victim (AV) relies on the facility for their care. According to an investigation, on or about March 6, 2026, Alleged Perpetrator 2 (AP2) called AV a “bitch” within the presence of AV, resulting in unreasonable emotional discomfort, which is a violation of resident rights, is neglect of care which constitutes verbal abuse as defined in 411-020-0002(1)(d)(A)(i). The facility failed to protect AV from verbal abuse, which is violation of Oregon Administrative Rules.
