Findings
The Alleged Victim (AV) relies on the facility for care, services and to provide a safe environment. According to an investigation, Alleged Perpetrator 2 (AP2) would live House A, where AV resides, unattended for extended periods of time creating a risk of serious harm to AV. The facility failed to provide care, services and a safe environment for AV, which is a violation of Oregon Administrative Rules. The allegation against AP2 was investigated and found to be substantiated for abuse.
