- The Alleged Victim (AV) resides at the facility and relies on the facility to meet his/her daily needs. AV was able to use their call pendant and would use it regularly to call for assistance. According to an investigation, on or about April 14, 2026, the Alleged Perpetrator 2 (AP2) removed AV's call pendant from around their neck and placed it under bed pillows, preventing AV from being able to call for assistance. This caused AV distress and resulted in unreasonable discomfort. AP2's actions are a violation of resident's rights, are considered neglect of care and constitutes abuse. The facility failed to provide a safe environment, which is a violation of Oregon Administrative Rules.
