Findings
The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, AV and Witness 1 (W1) have a history of resident-to-resident altercations. On April 25, 2026, AV was having behaviors when W1 approached and confronted AV to see what AV was upset about. AV and W1 started having a physical altercation that included hitting and punching each other multiple times. As a result of this altercation, AV sustained a broken nose. This was not the first altercation between AV and W1 that had resulted in physical violence. The facility failed to implement interventions into the service plans for W1 and AV to mitigate the resident-to-resident altercations, which is a violation of resident rights and is neglect which constitutes abuse.
