Findings
The Alleged Victim (AV) relies on the facility to provide a safe environment. On April 27, 2026, AV accessed the unsecured gate in the courtyard and left the facility without staff's knowledge. AV was found walking down the street by a staff member that was driving in their car. The facility failed to provide a secure environment for AV, which is neglect and constitutes abuse.
