Findings
The Alleged Victim (AV) is a known exit seeker and has eloped from the facility in the past. On or about May 25, 2026, AV was in the facility outdoor courtyard when the fire alarm went off and all doors unlocked. AV exited the courtyard gate and was found in the assisted living side of the facility. AV was not harmed; however, AV was placed at risk for harm during this event where AV was outside the locked facility and near a busy road. The Respondents failed to provide a safe environment. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
