Findings
On or about June 2, 2026, Alleged Perpetrator #2 (AP2) was feeding the Alleged Victim (AV) and was holding AV's wrists, preventing AV from moving. AP2's actions are a violation of resident rights, are considered neglect of care and constitute wrongful restraint, as defined by OAR 411-020-0002(1)(h)(A)(iii). The facility's failure to protect AV from wrongful restraint is a violation of Oregon Administrative Rules.
