Findings
The Alleged Victim (AV) is a known fall risk and has suffered five falls in less than two months. On or about June 26, 2026, AV suffered a fall and was found in his/her room on the floor. AV hit his/her face and suffered a laceration under his/her eye as well as bruising. Respondent failed to properly care plan for AV to reduce the risk of falls. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
