Findings
The Safety, Oversight and Quality Unit Facility review completed from October 25, 2020 – October 31, 2020, and incorporated into this notice by reference, substantiated the following: On or about October 25, 2020, the Department completed a visit and based on observations, interviews and record review, it was determined Respondent failed to implement adequate infection control practices to prevent the spread of COVID-19 (Coronavirus). Concerns discussed from this visit included resident cohorting, staff cohorting, improper use of Personal Protective Equipment (PPE), lack of proper hand sanitization, physical distance measures, lack of proper disinfection, monitoring for signs and symptoms of COVID-19, screening and testing. The Department completed a visit on October 27, 2020 and based on observations, interviews and record review, it was determined Respondent failed to implement adequate infection control practices to prevent the spread of COVID-19 (Coronavirus). Concerns discussed from this visit included: lack of screening, maintaining adequate staffing (two caregivers sent home with symptoms, Administrator on the schedule as caregiver, and no housekeeping staff), having designated staff for positive residents, resident cohorting, proper use of PPE, and proper disinfection. The Department completed a visit on October 31, 2020, and based on observations, interviews and record review, it was determined Respondent failed to implement adequate infection control practices to prevent the spread of COVID-19 (Coronavirus). Concerns discussed from this visit included: lack of screening, improper PPE use, resident and staff cohorting, and staffing.
