The facility’s failure to report positive cases to the department on May 17, 2021, substantiated the following: On or about June 4, 2021, the Department was notified by Oregon Public Health that the facility had a staff that had tested positive for COVID19 May 17, 2021. The facility failed to notify the Department or Oregon Public Health regarding this case. The facility's failure is a violation of Oregon Administrative Rules.
