Findings
On or about July 8, 2022, a complaint was received regarding residents living in an unlicensed facility. The facility thought they had a license, however, Safety, Oversight and Quality had not issued a license to the facility at that time. The facility having residents living in an unlicensed facility is a violation of Oregon Administrative Rules.
