Findings
On February 9, 2023, the re-licensure survey was completed, and the report was sent to the facility on February 16, 2023, with a due date for the facility’s plan of correction of March 3, 2023. On March 7, 2023, the Plan of Correction request was resent to the facility. On April 12, 2023, a letter was sent to the facility that the Plan of Correction was not accepted. The facility failed to appropriately respond to the request for a plan of correction, which is a violation of Oregon Administrative rules. Further rule violation(s) may result in civil penalties or further corrective action.
