- Based on observation and interview, conducted during a site visit on or about November 06, 2024, the facility failed to ensure fences surrounding the perimeter of the outdoor recreation area must be maintained in functional condition was substantiated. Findings include, but are not limited to: During an interview, Staff 1 (ED) stated the fence had been in the process of being repaired and replaced. Observed at 09:25 am back courtyard area fence was not secure with missing spaces. Two construction persons had been working on the fence.
