- Based on observation, interview, and record review, conducted during a site visit on 04/08/25 and 04/09/25, the facility’s failure to assess all residents with a significant change of condition was substantiated for sampled resident. This posed an immediate jeopardy situation that could threaten the health, safety, and welfare of residents.
