- The licensee failed to be responsible for the facility and the quality of services rendered in accordance with OAR 411-054-0025; per complaint the residents records were removed from the facility, Medication Techs did not have documentation demonstrating competency, specific needs staffing contract was not followed, resident records were not preserved for the required three years, a resident was smoking in their room, and administration was aware of the situation, which is a violation of Oregon Administrative Rules.
