- Based on interview and record review, conducted during a desk audit on 03/06/25, the facility’s failure to provide records to the Department upon request was substantiated. A review of electronic communication showed the Department requested documents related to the facility's APS ABST alleged event date of 01/09/25 on 02/14/25. The facility did not provide all requested documentation by the requested due date of 02/14/25. A phone interview on 03/06/25, Staff 1 (Executive Director) acknowledged the records were not provided to the Department in a timely manner. The facility’s failure to provide records to the Department upon request was substantiated.
