- Based on observation and interview, conducted during a site visit on 07/07/25, the facility’s failure to provide a manually operated emergency call system in each toilet and bathing facility used by residents and visitors was substantiated for 1 of 1 sampled residents (#1).
Findings include, but are not limited to: Compliance Specialist (CS) was unable to interview Resident 1 or make observations as s/he was no longer in the facility.
The following observations were made: No manually operated emergency call system in the bathroom, where Resident 1 had resided. An interview with Staff 1 (Executive Director) was conducted. S/He indicated the facility was required to have a manually operated call system beside each resident's bed and in the bathrooms.
It was determined the facility’s failure to provide a manually operated emergency call system in each toilet and bathing facility used by residents and visitors was substantiated.
