Findings
Based on an interview and record review conducted during a site visit on 07/24/25, the facility's failure to provide a training program that includes methods to determine the competency of direct care staff through evaluation, observation, or written testing was substantiated for 1 of 3 sampled staff (#5). Findings include, but are not limited to: Documents reviewed confirmed no documented completion of demonstrated competency to perform safe medication and treatment administration unsupervised for Staff 5. During an interview on 07/24/25, Staff 1 stated the LPN would complete competencies with Staff 5 by 07/25/25. Findings were reviewed and acknowledged by Staff 1 on 07/24/25. The facility's failure to provide a training program that includes methods to determine the competency of direct care staff through evaluation, observation, or written testing was substantiated which is a violation of Oregon Administrative Rules.
