Findings
On 05/24/25 the date of incident at approximately 1730 hours it was determined that only two staff members were working swing shift one caregiver and one med tech. It was determined that the facility did not have qualified direct care staff sufficient to meet the scheduled and unscheduled needs of the residents. The facility’s failure is a violation of Oregon Administrative Rules.
