- Based on interview and record review, conducted during an investigation on 12/22/25, the facility failed to accurately document written, signed physician or other legally recognized practitioner orders in the resident's facility record for all medications the facility is responsible to administer. The facility’s failure is a violation of Oregon Administrative Rules.
