- Based on interview and record review, conducted during an investigation on 01/23/26, the facility’s failure to provide records to the Department upon request was substantiated. Findings include but are not limited to: A review of electronic communication, dated 08/07/25, indicated the Department requested documents related to the facility’s Acuity-Based Staffing Tool (ABST) alleged event date 08/05/25. The facility had not provided all requested documentation by the due date of 08/08/25. The facility's failure to provide records to the Department upon request is a violation of Oregon Administrative Rules.
