Findings
The facility allegedly failed to protect, report abuse and investigate all reports of abuse and suspected abuse for the Alleged Victim. Alleged Victim experienced injury falls on 10/10/25 and 11/15/26, however, those falls were not reported to the local Adult Protective Services office. An investigation determined this is a violation of Oregon Administrative Rules.
