Findings
The facility failed to develop, maintain, and implement an Acuity-Based Staffing Tool (ABST). During the review, inconsistencies were identified, and the facility was not staffing according to the levels indicated by the ABST or the posted staffing plan for the date reviewed, June 6, 2025. The investigation determined that this constitutes a violation of Oregon Administrative Rules.
