- Based on interviews and record review, conducted during a site visit on 04/02/26-04/03/26, the facility’s failure to implement policy and procedure to assure the prevention and appropriate response to any incident, to immediately report abuse or suspect abuse to the Department, AAA, or law enforcement agency, and to promptly investigate all reports of abuse or suspected abuse was substantiated, which is a violation of Oregon Administrative Rules.
