- Findings
- Based on interviews and record review from the 05/21/26 investigation, it was determined the facility failed to obtain required background checks for all subject individuals (SI). Investigation Key findings:
• On 05/19/26 at 03:15 a.m., the Background Check Unit closed Staff 4’s background check because fingerprints were not completed. The facility was instructed to immediately remove Staff 4 from work and submit a new application.
• By 12:12 p.m. the same day, facility emails confirmed Staff 2, Staff 5, and Staff 1 all knew Staff 4 could no longer work.
• Staff 2 reported that Staff 4 had been notified on 04/27/26 of the fingerprint deadline and was repeatedly reminded. Staff 2 also stated they informed Staff 1 and Staff 5 on 05/19/26 that Staff 4 must stop working but Staff 1 kept Staff 4 on the schedule anyway.
• Scheduling and timesheet records show Staff 4 continued working on 05/19/26 and 05/20/26, after the application was closed.
• Staff 1 stated they believed Staff 4 had submitted a new application, though no new application existed.
• Before the investigation concluded, the facility terminated Staff 4 for failing to complete fingerprinting.
The facility allowed Staff 4 to continue working after being instructed to remove them due to an incomplete background check. The facility’s failure to submit required background checks for all subject individuals (S1) is a violation of Oregon Administrative Rules.
