- Based on interview and record review, conducted during an investigation on 05/22/26, the facility’s failure to provide records to the Department upon request was substantiated. Findings include but are not limited to: A review of electronic communication, dated 12/02/25, indicated the Department requested documents related to the facility’s Acuity-Based Staffing Tool (ABST) alleged event date 11/21/25. The facility had not provided all requested documentation by the due date of 12/03/25. The findings were reviewed with and acknowledged by Staff 1 on 05/22/26. The facility’s failure to provide records to the Department upon request is a violation of Oregon Administrative Rules.
