- Based on interview and record review, conducted during an investigation on 07/15/26 and 07/16/26, the facility’s failure to complete or update and review the Acuity-Based Staffing Tool (ABST) evaluation for each resident quarterly as required was substantiated. Findings include, but are not limited to:
- The facility’s census was 67.
- A review of the facility’s last ABST evaluation update per resident indicated 3 of 67 residents had not been updated quarterly as required as of 07/01/26.
- A review of a progress note dated 07/07/26 for Resident 3 and a Nursing Assessment dated 07/08/26 indicated Resident 3 was admitted to hospice, which is considered a significant change of condition. There was no documented evidence Resident 3’s ABST evaluation had been updated following the significant change of condition.
The facility’s failure to complete or update and review the Acuity-Based Staffing Tool (ABST) evaluation for each resident quarterly as required and/or is a violation of Oregon Administrative Rules.
