- Based on interview and record review, conducted during an investigation on 7/31/2026, the facility’s failure to provide records to the Department upon request was substantiated. Findings include, but are not limited to: A review of electronic communication dated 2/7/2025 indicated the Department requested documents related to the facility’s Acuity-Based Staffing Tool (ABST) alleged event date 1/5/2025. The facility had not provided all requested documentation by the due date of 2/8/2025. The facility’s failure to provide records to the Department upon request is a violation of Oregon Administrative Rules.
