- Allegation of facility neglect. During incontinent check, RV1 was discovered to have an injury to h/h left arm. RV1 was transported to hospital where it was discovered RV1 had a Closed Colle's fracture of the radius.After investigation, the following day, it was discovered the RV had an unreported fall the previous day.
