Findings
On May 24, 2016, Compliance Specialist (CS) interviewed Staff 2 who indicated Staff 1 was not available. Staff 2 indicated he/she is fairly new to the facility and is unaware facility having a calendaring systems to track Service Plan and renewal dates. Staff 2 indicated if the facility does not have one, he/she will create one. On May 24, 2016, CS interviewed Staff 3 who indicated there is a calendaring system in place to track Service Plan and renewal dates and Staff 1 maintains it. Staff 3 further indicated Staff 1 mails out letter to resident families to invite them to participate in Service Plan On May 24, 2016, CS interviewed Resident 1 who indicated he/she indicated they recall receiving an updated Care Conference Plan but did not recall participating in the Care Conference. CS asked Resident 1 if he/she ever participated in a Care Conference and Resident 1 indicated the only Care Conference he/she participated in was his/her initial move in assessment.On May 24, 2016, CS interviewed Witness 1 via telephone. Witness 1 indicated he/she asked for copies of last two Care Conferences because he/she noticed subtle changes to how staff were bathing Resident 1. Witness 1 stated the facility never responded to her requests. CS asked Witness 1 if he/she was aware of any Care Conferences taking place in February and May 2016. Witness 1 indicated he/she was unaware of any Care Conferences taking place during those times. Witness 1 indicated he/she could not understand how they did not receive word on the Care Conference in May 2016. Witness 1 indicated he/she dropped off a letter to Staff 1 on May 9, 2016 indicating the family ' s vacation plans May 9th May 15th; Witness 1 indicated he/she left emergency contact numbers on the letter for Staff 1 in same letter. On May 31, 2016, CS interviewed Staff 1 and informed him/her of information CS obtained from May 24th interviews. CS further informed Staff 1 that CS requested and received Care Conference Plans for Resident 1 and 2 Sample Residents. None of the Care Conference Plans received had any family members or residents ' signatures on them.
