Findings
On July 29, 2016, Compliance Specialist (CS) interviewed Witness 1 who stated the week before, Resident 1 was transported to medical services without supervision. Witness 1 indicated Resident 1 has cognitive issues and was found wandering the halls unsure of his/her destination and what he/she was supposed to be doing. Witness 1 further indicated he/she called the facility administration twice to determine what was going to be done about it. Witness 1 reports no solution has been offered by the facility. Witness lastly stated, when the incident occurred, Resident 1 was being transported for tests he/she did not need because those test were done twice the previous week. On July 29, 2016, CS interviewed Resident 1 who recalled last week ' s incident. Resident 1 stated he/she was confused and lost, wandering around a medical facility he/she has been to before. On July 29, 2016, CS reviewed Resident 1 ' s care plan which indicates Resident 1 is able to make day to day decisions. However, Resident 1 can be forgetful and has periods of confusion.On July 29, 2016, CS interviewed Staff 2 who stated he/she agrees with care plan assessment of Resident 1 having Dementia with Schizophrenic tendencies.On July 29, 2016, CS interviewed Staff 1 who stated Resident 1 has no family, no guardian so Staff 1 believes Resident 1 ' s case manager should accompany Resident 1 to medical appointments because the facility does not have the man power to help with the transport. Staff 1 believes Resident 1 is still an appropriate resident for an assisted living facility and insisted the facility would never keep a resident who requires more intensive care like a memory care center. Staff 1 stated he/she would be more than happy to talk to the doctor ' s office to attempt to coordinate something workable.
