- The allegation that the facility failed to have policies and procedures in place to assure the prevention and appropriate response to any incident to include injury of unknown cause. The facility failed to respond appropriately by re-assessing, monitoring or reviewing medications in accordance with OAR 411-054-0028(1) per complaint that a resident had several unwitnessed falls that were not reported to family per service plan, and there was no mitigation to prevent excessive falls by re-assessment or monitoring for decline was verified.
