The allegation that the facility failure to have qualified awake direct care staff, sufficient in number to meet the 24-hour scheduled and unscheduled needs of each resident per OAR 411-054-0070(1) as stated in complaint that residents are leaving the facility out a back gate, not being assisted with mobility and medications are not being given was verified.
