Findings
Based on interview and record review, it was determined the facility failed to adopt an Acuity-Based Staffing Tool (ABST) tool to determine appropriate staffing levels for the facility. The ABST was unable to produce staff time required to building a staffing schedule. An investigation determined this is a violation of Oregon Administrative Rules. Corrective Action taken on related allegation.
