The facility failed provide services to assist the resident with mobility including one-person transfers in accordance with the OAR 411-054-0030(1)(e)(A) Per complaint, a resident asked for help with transfering and staff asked them to wait as the staff was busy with another task, resulting in the resident attempting to self-transfer and the fall.
