The allegation that the facility failed to provide enough qualified awake direct care staff, sufficient in number to meet the 24-hour scheduled and unscheduled needs of each resident in accordance with OAR 411-054-0070(1) per complaint that staff did not have enough time to go back and check on a resident due to the lack of staffing was verified.
