The facility failed to exercise reasonable precautions against any condition that may threaten the health, safety, or welfare of the residents in accordance with OAR 411-054-0025(4) per complaint that staff do not wear masks in the facility and there is box screens and/or mattress leaning against the wall in residents room causing a potential trip hazard and there are chemicals in an open and unlocked salon.
