Findings
On or about 7/18/2022 the Alleged Victim (AV) was transitioning from one medication to a new medication. Staff was instructed via text from RN to discontinue a medication and give new medication. Staff gave discontinued medication and not the new medication. The facility failed to provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
