Findings
The facility failed to provide direct care staff sufficient in number to meet the scheduled and unscheduled needs of the residents in accordance with OAR 411-054-0070(1) per complainant that it is extremely difficult to get ahold of facility staff to discharge medical stable residents back to the facility.
