Findings
The facility failed to provide direct care staff sufficient in number to meet the scheduled and unscheduled needs of the residents in accordance with OAR 411-054-0070(1) per complainant that the facilities only have one med tech and one caregiver covering all three floors. It took one hour before staff answered their call light.
