The facility failed to develop a service planning team that consists of the resident and any person of the resident's choice per complaint that resident's family tried over a year to coordinate a care conference, and the family learned that the care plan was updated with no input from the family or the resident. The facility failure is a violation of Oregon Administrative Rules.
