Findings
On or about March 05, 2024, a review of the facility's ABST indicated the facility used the Oregon Department of Human Services ABST; the facility's census was 41 residents. 41 out of 41 residents were entered into the ABST, however 25 of 41 resident profiles had not been updated quarterly; Staffing levels generated by the ABST indicated the facility required four care staff on day shift, four care staff on swing shift, and two care staff on night shift, which matched the facility's posted staffing plan. It was confirmed the facility failed to update an Acuity-Based Staffing Tool (ABST).
