Findings
On or about June 1, 2023, the facility failed to provide three daily nutritious, palatable meals with snacks available seven days a week, by serving residents food that was not within appropriate temperatures and some of the food was dry and residents were unable to cut the food up. The facility’s failure is a violation of Oregon Administrative Rules.
