- The facility failed to implement the resident's right to receive proper notification if requested to move-out of the facility, and to be required to move-out only for reasons allowed and have the opportunity for an administrative hearing. The facility failed to implement resident's rights to receive proper notification for move-out, was denied readmission and transferred to another facility for 2 of 2 sampled residents. An investigation determined this is a violation of Oregon Administrative Rules.
